Welcome to Oculo Facial Clinic Tokyo (“we,” “us,” “our,” or “Clinic”). By accessing or using our website, seeking consultation, or receiving any services from us (including surgical, medical, diagnostic, cosmetic, or ancillary services), you (“you,” “patient,” or “client”) agree to these Terms of Service (“Terms”). If you do not agree, you should not use our services or website.
1. Scope & Applicability
1.1 These Terms govern your use of the website, communications, appointments, consultations, treatments, surgeries, and other services provided by the Clinic.
1.2 Supplementary agreements, consents, or documents (e.g., patient consent forms, privacy policy, surgical consent) may also apply and control to the extent of any conflict.
1.3 We reserve the right to modify these Terms at any time; modifications become effective when posted. It is your responsibility to check for updates.
2. Services Offered
2.1 The Clinic offers medical, surgical, diagnostic, cosmetic, and reconstructive services relating to periorbital, eyelid, orbital, and facial structures (e.g. oculoplastic surgery, orbital surgery, eyelid cosmetic procedures, reconstructive surgeries, injectables, etc.).
2.2 The exact scope of services, risks, benefits, alternatives, expected outcomes, and precautions will be provided in patient-specific consultations and in the consent documentation.
2.3 We may decline to provide services to any individual, at our discretion, based on medical assessment, risk, or capacity.
3. Patient Eligibility & Obligations
3.1 You represent that all information you provide (medical history, medications, allergies, prior surgeries, etc.) is accurate, true, and complete.
3.2 You agree to follow all instructions given by the Clinic (pre- and post-operative, medication, follow-up, etc.).
3.3 You will notify the Clinic immediately of any adverse symptoms, complications, or concerns.
3.4 You consent to necessary diagnostic tests, imaging, photography, and evaluations as needed for care.
4. Appointments, Cancellations & Fees
4.1 Appointment bookings (in-person or virtual) require your consent to these Terms and payment of any applicable fees or deposits.
4.2 Once the surgery/treatment deposit is paid, you have up to 8 days to request a refund. After 8 days, no refunds will be issued.
4.3 Payment for surgery must be made in full before or on the day of surgery. Failure to pay in full by the day of surgery may result in cancellation of the procedure.
4.4 You are responsible for paying all fees for services rendered, including consultations, diagnostics, surgical fees, anesthesia fees, facility fees, medications, implants, materials, and other ancillary charges.
5. Payment, Billing & Insurance

5.1 Payment is due according to the schedule set forth in your treatment plan or agreement.

5.2 Insurance (covered treatments):

For insured patients, treatment costs are subject to Japanese health insurance laws.

Patients are responsible for paying their legally determined self-pay limit (“patient contribution limit”), which depends on their income category.

Example insured treatment costs include:

Ptosis surgery (levator resection): approx. ¥60,000
-
Entropion surgery: approx. ¥30,000
-
Orbital fracture repair: approx. ¥70,000–¥200,000 (depending on severity and laterality)

Additional costs apply for general anesthesia (approx. ¥20,000).
5.3 Self-pay / out-of-pocket treatments (cosmetic or elective procedures): -
For self-pay procedures, patients are responsible for the full fee as published in the Clinic’s current fee schedule.

A reservation deposit of ¥110,000 is required at the time of booking surgery. This deposit is non-refundable after 8 days.

Upper/lower eyelid corrective surgery: ¥330,000 – ¥1,210,000 (depending on surgeon)

Orbital fat repositioning (Hamura method): ¥330,000 – ¥1,210,000 (depending on surgeon)

Orbital tumor removal: ¥440,000 – ¥1,980,000

Lacrimal duct bypass surgery: ¥1,320,000 – ¥1,980,000

Eyelid ptosis + orbital fat repositioning (combined): discounted by ¥220,000

Hyaluronic acid injection: ¥95,700 per vial / ¥249,700 per session

Botox injection: ¥33,000 per session

Discounts for monitor patients (20% off) apply if the patient consents to photographs/videos being used for educational or promotional purposes (separate consent required).

5.4 All payments must be completed before treatment. Unpaid balances may delay or cancel scheduled procedures.

5.5 Acceptable payment methods are Credit Card (via Stripe), PayPal, Bank Transfer, or Cash. For international patients, payment must be received in full before we can confirm in-person consultation and surgery dates.

5.6 Refund Policy:

Insured treatments: no refunds beyond the official patient responsibility under Japanese health insurance law.

Self-pay treatments: Deposits are refundable only if canceled within 8 days of payment. After 8 days, deposits are non-refundable.
6. Risks, Complications & No Guarantee
6.1 All surgical and medical procedures carry inherent risks, including but not limited to infection, bleeding, scarring, asymmetry, vision changes, anesthesia complications, nerve injury, implant complications, or disfigurement.
6.2 While we aim for optimal outcomes, we cannot guarantee any specific result, perfect aesthetic symmetry, or zero complications.
6.3 If complications occur, additional care may be necessary; you agree to cooperate with follow-up.
6.4 We are not liable for any loss, damage, or cost arising from risks that were disclosed and accepted in your consent.
7. Consent & Shared Decision-Making
7.1 Before any procedure, you must sign informed consent documents confirming you understand the procedure, alternatives, risks, and post-operative care.
7.2 You have the right to ask questions, seek second opinions, or refuse/withdraw consent at any time (subject to safety).
7.3 In cases of co-management or referrals, you agree that we may share medical information with other providers as necessary for your care (with your authorization).
8. Privacy, Medical Records & Confidentiality
8.1 Your personal health information will be treated in accordance with our Privacy Policy.
8.2 We will maintain medical records, surgical notes, imaging, photos, and documentation in a secure manner.
8.3 We may use anonymized or de-identified photographs or data for academic, teaching, or research purposes, but only with appropriate consent or in compliance with applicable laws.
8.4 We reserve the right to disclose information when required by law, court order, or public health agencies.
9. Photography & Filming Policy
9.1 Patients and visitors are strictly prohibited from photographing, video recording, or audio recording inside the Clinic without prior written consent from the Clinic.
9.2 Recording or photographing staff, medical procedures, or facilities is not permitted without explicit Clinic authorization.
9.3 Under no circumstances may you film, photograph, or record other patients without their express written consent. Any violation of this policy may result in immediate removal from the premises and potential legal action.
10. Intellectual Property, Website Use & Content
10.1 The content on our website (text, images, logos, videos) is owned or licensed to the Clinic; you may use it for personal, non-commercial purposes only.
10.2 You agree not to reverse engineer, reproduce, distribute, or misuse content without permission.
10.3 If you submit any feedback, suggestions, or content, you grant us a non-exclusive, royalty-free license to use, reproduce, and adapt it.
11. Limitation of Liability & Indemnification
11.1 To the maximum permitted by law, the Clinic, its staff, directors, and affiliates are not liable for indirect, incidental, special, or consequential damages arising from your use of services or procedures, even if advised of the possibility.
11.2 Our total liability (if any) for direct damages shall be limited to monies you actually paid for the relevant service.
11.3 You agree to indemnify, defend, and hold harmless the Clinic, staff, and affiliates from any claims, liabilities, costs, or losses (including attorney fees) arising from your breach of these Terms or your negligence.
12. Governing Law, Dispute Resolution & Jurisdiction
12.1 These Terms are governed by the laws of Japan.
12.2 Any dispute arising hereunder shall first be attempted to resolve by good faith negotiation or mediation.
12.3 If unresolved, disputes shall be submitted to the courts of Tokyo, Japan.
13. Severability & Waiver
13.1 If any provision of these Terms is held invalid or unenforceable, that provision will be severed and the remainder will continue in full force.
13.2 No waiver of any term or breach shall be deemed a further waiver or continuing waiver, unless given in writing by authorized personnel.
14. Notices & Communications
14.1 We may send you notices via email, postal mail, or posted on our website.
14.2 You agree to receive communications (appointment reminders, follow-up notices) through those channels unless you opt out where allowed.
15. Miscellaneous
15.1 Force Majeure: We are not responsible for failure or delay due to causes beyond our control (natural disasters, government actions, etc.).
15.2 Assignment: You may not assign or transfer your rights under these Terms without our written consent; we may assign to a successor.
15.3 Entire Agreement: These Terms, along with any supplementary agreements or consents you sign, constitute the entire understanding between you and us concerning the services.
15.4 All fees, terms, and policies described herein are subject to change at the discretion of the Clinic.
